Emergency Medical Release
Authorization for emergency medical care when a parent or guardian cannot immediately be reached.

This document is issued by PassionFlower Marketing LLC, a Florida limited liability company d/b/a Happy Place Craft Space ("Happy Place," "we," "us," or "our"). It supplements, and should be read together with, our Terms of Service, Privacy Policy, Refund Policy, and Craft Host Agreement.
1. Emergency authorization
In the event of a medical emergency involving my child during a Happy Place activity, I authorize the Craft Host, on-site staff, and emergency responders to:
- Administer basic first aid.
- Call 911 or other emergency services.
- Arrange transport to the nearest hospital or urgent care.
- Consent to emergency medical treatment, including x-rays, medication, or surgery, as recommended by a licensed provider, if I cannot be reached in time.
2. Medical information you provide
I will disclose known allergies, medical conditions, medications, and dietary restrictions at registration. I understand that Craft Hosts are not medical professionals and cannot administer prescription medication, EpiPens, or insulin unless separately arranged in writing.
3. Financial responsibility
I am solely responsible for all costs of medical evaluation, treatment, transport, and follow-up care. Happy Place, its Craft Hosts, and venue partners are not responsible for medical bills.
4. Limits of this release
This authorization is limited to the specific activity my child is attending and expires at the end of that activity. A new authorization applies for each program or booking.
5. Emergency contact
I will provide a primary and backup emergency contact at registration and will keep those numbers current.
6. Governing law
This Release is governed by Florida law.
