Safety Incident Report
How to report an injury, near-miss, or safety concern that occurred during a Happy Place activity.

This document is issued by PassionFlower Marketing LLC, a Florida limited liability company d/b/a Happy Place Craft Space ("Happy Place," "we," "us," or "our"). It supplements, and should be read together with, our Terms of Service, Privacy Policy, Refund Policy, and Craft Host Agreement.
1. When to report
File a Safety Incident Report as soon as reasonably possible, and no later than 24 hours after any of the following:
- Any injury to a child, adult, Host, or bystander that required first aid, medical evaluation, or 911.
- A near-miss that could reasonably have caused injury (e.g., broken glass, tool malfunction, unsafe venue condition).
- A behavior concern involving a child, adult, or Host that could affect safety.
- Damage to a venue or third-party property.
- Suspected abuse or neglect (also follow mandatory reporting laws where applicable).
2. How to report
Email support@happyplacecraftspace.com with the subject line "Safety Incident Report" and include the information below. In a life-threatening emergency, call 911 first, then notify Happy Place.
3. Information to include
- Date, time, and location of the incident.
- Name of the Craft Host and any staff or volunteers on site.
- Names and ages of anyone involved or injured (children identified only to the extent needed).
- Description of what happened, in factual terms, including what was being made or done.
- Any first aid or medical care provided, and whether 911 was called.
- Names and contact information of witnesses.
- Photographs of the scene, if safe to take and if not restricted by venue policy.
- Actions taken to prevent recurrence.
4. Mandatory reporting
Craft Hosts are expected to comply with all applicable state and federal mandatory-reporting laws, including reports of suspected child abuse or neglect. Reporting to Happy Place does not replace legally required reports to authorities.
5. Confidentiality
Reports are shared only with the Happy Place staff needed to investigate, insurers, and, where required, law enforcement or regulators. We do not retaliate against anyone who makes a good-faith report.
6. Follow-up
Happy Place will acknowledge receipt of each report within two (2) business days and communicate any required next steps, including whether the incident affects the Host's marketplace status.
